Pain to Gain is a Body attribute perk found in Cyberpunk 2077’s perk tree, introduced as part of the 2.0 overhaul that reshaped how Attributes and Perks work. It upgrades an effect you already have if you’ve invested in Adrenaline Rush, one of the signature combat perks in the Body tree.

Rather than functioning independently, Pain to Gain acts as a bonus layered on top of Adrenaline Rush. It rewards players who are already leaning into an aggressive, kill-to-heal loop.

While using Adrenaline Rush, Pain to Gain perk helps speed of the all recharging items improved by 20%. In practice, this means that every kill you land while Adrenaline Rush is active shortens the wait before you can use a health item again.
