OSRS Maggot King Strategy: Gear, Mechanics, and How to Melee Punish

The Maggot King (some players just call him the OSRS Worm Boss, and honestly, fair) is the newest boss in OSRS, and he drops a solid new crush spec weapon plus a best-in-slot ranged amulet. You need to do The Blood Moon Rises quest and do some prep work to get to him. So what’s the best OSRS Maggot King strategy? This OSRS guide covers the gear and every melee punish method the community has figured out so far. So stick with me.

Maggot King Boss Fight Requirements
You need to finish The Blood Moon Rises quest before going for this boss, and aim for the stats below:

Before you head in, grab a hammer, saw, magic stone, some bronze bars, adamant bars, ironwood planks, and nails. Why? Because you’ll use these to build a bank chest near the entrance, which saves you from running back to a real bank between trips. A stamina potion wouldn’t hurt, either. You’ll get tired soon.
How to Get to the Maggot King Location
DON’T skip this part! You can teleport to the boss. You can thank me later. But here’s how to get it:
South of the portal, you’ll find an NPC getting swarmed by three mosquitoes. Kill the mosquitoes and talk to him. This unlocks a teleport shortcut that drops you much closer to the boss entrance, past the annoying stuff in town.

If you prefer the normal path through the town, follow the steps below:
- Teleport to Castle Drakan using Drakan’s medallion (it has a new teleport option straight into the castle). You can also get here through Darkmeyer if you want.
- Go through the portal into the Vampyrium realm.
- Run east and build your bank chest near the blood fountain. Gear up here.
- Run south and use the NPC you saved to teleport closer to the boss.
- Head north through the forest and squeeze through the bush near the two magicians.
- Now you’re at the Darkwood Trees entrance. Walk in.
Best Maggot King Gear Setups
You can start the fight with a magic or ranged build, but you have to switch to melee in the last phase. I recommend the magic build, as Maggot King’s weakness, more than anything, is fire.
Magic Setup
The Maggot King has 80% weakness to fire spells. They also apply a stacking burn effect, which slows their healing from larvae. So I think magic is the strongest overall damage option, budget or not.

Range Setup
If you don’t have the Confliction Gauntlets, range can be the better choice over magic. It’s not as strong overall, but it’s more consistent without that Gauntlet build.

Melee Setup
Melee is mandatory once the fight hits its melee phase, so don’t skip this switch regardless of your main build.

Note: Make sure your weapon is actually set to crush. The Zombie Axe and Scythe both default to other styles sometimes, so double-check before you walk in.
For your spec weapon, bring whatever defense-lowering spec you’ve got and slap him with it at the start of the melee phase. Elder Maul, Dragon Warhammer, Crimson Kiss, or Accursed Sceptre all work fine. Lastly, bring the inventory & Prayers below:
- Pray Protect from Missiles before you even enter, since the boss opens with a ranged attack immediately.
- Bring a way to cure poison (sanfew serum or antidote works).
- Bring prayer restores, since you’re flicking prayers the entire fight.
- Hard food is enough. Brews aren’t really necessary here.
- Pack an emergency teleport and a way back to the boss.
- If you’re maging, bring fire runes/staff charges. If ranging, bring your ammo plus a way to death charge if you’re using it.
Maggot King Attacks Explained
Maggot King has three types of attacks. Read them below to know how to deal with them more easily.
Ranged & Magic Auto Attacks
The boss fight begins the second you walk in with ranged combat. You should have Protect from Missiles to counter this. His auto attacks launch green (ranged) or yellow (magic) spit at you. On impact, it splits into three projectiles: one lands right on your tile, and two more scatter in the 3×3 area around you.
Those green pools are poisonous. So just move two tiles away to avoid all of it. And even if you couldn’t dodge them, at least don’t stand on them. Easy!
Those splats stick around on the floor until something clears them. The green ones cut 45 seconds off your antipoison timer, and the yellow ones delay your next attack and lower your stats a bit. Neither one does much damage on its own, but stacking a bunch of pools makes the arena a lot harder to move through, so try to avoid them anyway.

Screech and Carrion Swarm
Periodically, the boss screeches three times in a row. If you’re still praying overhead, you’ll get 15-25 damage per hit for all three, plus an attack delay. Sp the second you hear or see the screech (a white aura shows up around him), turn off your prayer completely.
Screech always comes with Carrion Swarm. Piles of Carrion rain down on random tiles. First impact has up to 25 damage, and after that, up to 10. So scream and run away from the first shadows you see. But Carrion is not completely useless. They clear old acid pools off the floor, so you actually get a bit more room to move right after.
Every screech changes the boss’s attack style. That means if he is using ranged, he switches to magic next, and vice versa. So keep your eyes peeled for what he used last so you know what to pray when the next screech ends.

Ur-Maggot Larvae (~83% HP)
Once he is below 1250 HP (roughly 83%), every Carrion Swarm also launches an Ur-maggot larva into the arena from one of the nearby corpses. Shoot it with a standard bow (arrows only, not crossbows or bolts) before it lands, and it dies instantly with no delay. This also works to proc Death Charge if you’re using it.
If you let it land, it picks a row and shuffles left and right for the rest of the fight. It eats any Carrion in its path and heals the boss 1 HP per pile consumed, which isn’t much. If you’re standing on its tile, though, it does rapidly ramping damage to you that gets converted straight into boss HP, so don’t just stand there.
Most players only bother killing the larva in the front row since it’s the one most likely to get in your way. Ignoring the rest is completely fine, because the healing just isn’t significant enough to chase them all down.

OSRS Maggot King Melee Phase Strategy
At 980 HP (roughly 65%), he goes, “To hell with this magic BS. Let’s fight like real men.” In other words, starts his melee phase. He moves up to the bone wall and starts slamming his head down in a 3×5 AoE in front of him, alongside more bile splats.
The slam does a lot of damage if you get caught in it. Dodge by stepping to the side, out of the AoE, and melee attack from there. Swap into your melee gear the moment he screeches into this phase, and pop your defense-lowering spec right away for the burst DPS.
He doesn’t attack the same way forever, though. Once his HP drops low enough (roughly 250-300), the melee phase becomes permanent, and he stops switching back to ranged/magic entirely. Here’s how to deal with that:
Melee Punish Method 1: Step-Back Double Tap
This is the most common technique. When the boss slams, his head/neck fully extends for a split second. Click him the exact tick it extends, and you get a bonus attack with no cooldown, on top of your normal attack cycle.
To keep this going, every slam, step one tile away as he slams, click him for the punish hit, then click him again right after to get your normal attack going. Some players prefer stepping in the opposite direction each time (walking side to side), which is a bit easier to land but slightly slower DPS since the boss has to reposition to follow you.

Melee Punish Method 2: Head Snipe
Rather than punishing his body, this method targets the head itself for a completely separate free hit. Position yourself at melee range from where the head stretches out, then click to attack right as it extends. If you dodge too early, you’ll need to wait a tick before you can land it, so timing your dodge close to the extension is key.
This head hit doesn’t affect your regular attack cycle at all; it’s a genuinely free hit on top of everything else. Just remember to click back onto the boss’s body afterward, or you’ll stop attacking entirely. This method takes more practice than the other two, but it lets you land damage on both the head and body without one interrupting the other.
Maggot King Drops
Straight up, this isn’t a boss you fight for guaranteed profit. The loot table is a lot like The Nightmare’s: if you’re not walking away with a unique, you’re probably losing money on supplies most times. That said, the uniques are genuinely worth chasing:
- Fangs of venenatis combine into the Necklace of Rapture, A.K.A. Amulet of Rapture, the new best-in-slot ranged amulet.
- Crimson kiss a new crush-category special attack weapon.
- Dull ancient medal and 2-4 Stymphike tartares (common drops that restore HP and prayer and give a combat stat boost). They’re not big money, but they stretch your trips out longer since you’re not consuming food and prayer pots as fast.
If you ask me, I would say fight it if you want the unique stuff or the pet. Skip the headaches if you’re just after GP. It’s not worth your time. But the choice is yours.
