To unlock the (Don’t Fear) The Reaper quest, you need to complete the Chippin’ In side job before reaching the point of no return and choose specific dialogue options when talking to Johnny Silverhand. When you go to the Oilfields with Johnny, he will start another conversation near the mission objective, and you must select the following dialogue choices:

- Choose “The Guy Who Saved My Life”
- For third response, select “No, you f**ked that up too.”
- For the next two dialogues, choose these items respectively: “What do you want from me” and “OK. But as second chances go, this is your last”

How to Get (Don’t Fear) The Reaper Ending
After meeting Hanako during Nocturne Op55N1, you will sit with Johnny on the rooftop at Embers. There, you need to suggest that Johnny and Rogue go to Arasaka Tower. When Johnny asks if you are sure, you must stay indecisive.
Instead of picking one of the dialogue options, you need to wait for five real-time minutes without doing anything. After this, Johnny will suggest going in alone on a suicide run. In this regard, (Don’t Fear) The Reaper will become available.

When you connect Alt to the mainframe, you should head to the Mikoshi Access point. Follow the waypoint marker until you reach a locked door. After Alt briefly becomes unresponsive, you will need to force the door open.

As you attempt to lift the door, Adam Smasher will attack you. He is the final enemy in this mission and must be defeated to move on to the Mikoshi Access Point. After defeating him and reaching the access point, you have to connect yourself to Mikoshi and put your body into the coolant.
You will be sent back to the rooftop at the beginning, where you need to pick up a necklace from the spot where you spoke with Misty. You will then meet Johnny again, who congratulates you on carrying out the plan successfully. He also explains that Alt has already separated you and Johnny by using Soulkiller on V.
At the end, Alt gives you two choices:
- Return to V’s body by entering the well.
