How to Find NMS Reality Glitch in Expedition 23

Expedition 23 is deliberately digging up old mechanics and Stabilised Reality Glitches (those weird little exotic trophies from the Visions update years ago) are one of them.
The Visions milestone wants you to grab one, and once you do, the Space Anomaly unlocks. Simple on paper, until you’re standing on a planet wondering why your scanner is gaslighting you.
The good news is you don’t actually have to go hunting an exotic planet from scratch if you don’t want to. There’s a fast way, the official way the expedition wants you to do, and a solid fallback when the game decides to be weird.
What You’ll Need:
The Fast Way Most People Should Just Do
As soon as your ship is repaired and you can leave the starting planet, open the milestone list and select Fractal. Fly to the nearest scorched/charred planet, pull out the Analysis Visor and start scanning creatures.

You’re looking for one whose body temperature is over 45°C. Once you find it and complete Fractal, the game hands you Reality Glitches as a reward. Those glitches will satisfy Visions right away and unlock the Space Anomaly.
The Proper Expedition Route
If you want to do it the way Hello Games intended, just select Visions itself as your active milestone. It should give you a marker that leads toward a planet with a Capillary Shell (one of the classic Reality Glitches).

Land, use the normal Analysis Visor, look for the question-mark icon, walk up to it, pick the thing up. After you collect it, follow whatever the milestone still wants. Sometimes you need to hop back in your ship and get the usual developer communication before it fully registers as done.
When the Marker Lets You Down (Fallback)
Sometimes the Visions route dumps you in a system that just doesn’t have anything useful, or the glitch refuses to show up.
When that happens, a bunch of players have had success going back to the bubbling planet (or the little moon) in the Rendezvous 1 system. The bubble collectibles there count. Other people have grabbed an Electric Cube off a Shattered planet and that worked too. Basically, any exotic world can work if the guided route is being unhelpful.

Once you’re on the right kind of planet, the method is the same as it’s always been. Pull out the regular Analysis Visor and scan around. You’re looking for the ? icon. Stabilised Reality Glitches show up within about 100 units. Walk over, hit the pick-up prompt and you’re done.
One really important Expedition-specific heads-up: don’t lean too hard on Target Sweep. A lot of people are reporting that Target Sweep either points them forever in one direction or gives a signal so weak it’s useless. The normal Analysis Visor and the question-mark marker are still the reliable way.
Which Glitches Actually Count?
Visions doesn’t seem to care which specific Reality Glitch you get. You can finish it with Electric Cubes, Cable Pods, Capillary Shells and others. As long as it’s a Stabilised Reality Glitch, it should clear the milestone.
After You’ve Got One

You can stick the glitch in a base later if you want (Build Menu, then go to Decorations / Glitches). If you keep one in your inventory you can place extra copies by spending Pure Ferrite. Completely optional, though, has nothing to do with finishing Visions.
Quick Reality Checks If Things Are Being Weird
Make sure the right milestone is actually selected as active. Prefer the normal Analysis Visor over Target Sweep. If other players’ bases or terrain edits are messing with spawns, a lot of people just turn multiplayer off for the expedition, it’s a common community workaround because these seasons can get a bit chaotic.
And if Fractal’s reward doesn’t instantly pop Visions for some reason, just fall back to picking one up the old-fashioned way. Both routes worked for me.
Once Visions is done, the Anomaly is yours and you can keep rolling through the rest of this big anniversary expedition. It’s basically a playable museum of the last ten years, so enjoy the nostalgia trip.
