The song is basically about survival at pretty much any cost. The Surgeon sees life as a constant race where any weakness has to be hidden and you shouldn’t make any mistakes either. In his mind, you should remember that even friends can become disposable if they get in the way and become a liability.
Bury or Die sums up that mindset: stay ahead and leave your past behind before it drags you down. But the ending of the song is more like a warning than an actual victory.
